PCCC is the first clinic in the state of Hawaii that focuses on Long COVID and services not only patients living on Oahu but on the Neighbor Islands as well in the South Pacific. PCCC is nationally recognized and draws on the clinical expertise of our staff to assist patients in their recovery. PCCC works with providers across the Queen’s Medical Center to offer comprehensive and holistic care utilizing multiple specialties including, but not limited to, pulmonology, cardiology, neurology, and physical therapy. PCCC collaborates with scientists at the John A. Burns School of Medicine to remain knowledgeable about cutting edge treatments and clinical recommendations and is a member of the IDeA States COnsortium for clinical REsearch (ISCORE) Network that participates in the RECOVER Initiative to learn about the long-term effects of COVID. What Does Post-COVID Mean? Post-Covid (or long COVID) is broadly defined as signs, symptoms, and conditions that continue or develop after an acute COVID-19 infection. PCCC focuses on signs and symptoms that are continuing 30-45 days after an acute infection. Symptoms include: Dyspnea or Dyspnea on exertion Fatigue Cognitive impairment or "brain fog" Headache or cluster headache Arthralgia Myalgia or chronic fatigue & malaise Lightheadedness or dizziness Reduced or impaired mobility Impaired daily function or problems related to life management difficulties Ansomia Dysgeusia Patient Eligibility Patient must have a confirmed positive COVID-19 infection diagnosis and copy of test results. Patient must be at least 30 to 45 days post COVID infection with long haul symptoms. MUST BE REFERRED BY CURRENT PCP Patient must be 15 years of age Download and complete ALL sections of the ”Provider Referral Form” INCOMPLETE FORMS WILL NOT BE PROCESSED. Refer a Patient Insurance Information Most insurance plans are accepted, except the following: Medicare HMO Commercial HMO Kaiser health plans No Fault Seoul Medical Group (SMG) Tricare Prime Questions? A referral from the patient’s primary care provider is required for all services. How to refer a patient: Review and complete the referral form. Provide demographics sheet with insurance information. If a prior authorization is necessary; please obtain and include with referral packet. Provide the most labs and office visit notes. Office visit notes must have the ICD-10 codes from the referral form documented.
